Mountain Family targets the more than 34,000 persons in this area (25% of the population) who have low household incomes (defined as = 200% of the federal poverty guideline [FPG]). More than 22,400 persons in the region call Mountain Family their healthcare home. For this patient panel, 83% of have low incomes, 43% are uninsured, 30% are enrolled in Medicaid, and 60% are Hispanic/Latino. The organization operates four integrated health center hubs and six school-based health centers across the West Mountain Region. Mountain Family’s mission is “We improve the health of our community by providing equitable and affordable medical, behavioral, and dental healthcare for all.” Mountain Family plans to utilize this CDS award to reconfigure, renovate, and improve an existing medical care facility located in the Town of Gypsum, in western Eagle County, for the provision of integrated primary care to nearly 3,000 residents of that area who are not currently served by a safety net clinic. Owned by partner Vail Health, the facility will be leased to Mountain Family on a long-term basis. The new Gypsum Integrated Health Center will include four med
ical exam rooms, one behavioral health room, three dental operatories, medical prep and laboratory room, pharmacy dispensary, and space for team-based care and patient support services. The project start date is July 1, 2022, with completion and opening of the health center scheduled for July 1, 2023. By May 31, 2026, Mountain Family will serve 2,500 patients annually in the new facility.
