Community Project Funding/Congressionally Directed Spending - Construction - Address: OCHIN, Inc., PO Box 5426, Portland, OR 97228 Project Director: Jennifer Stoll, EVP, External Affairs Phone: (503) 943-2500 Email: stollj@ochin.org Website: www.ochin.org Funds Requested: $500,000 OCHIN proposes a three-year project to implement a collaborative partnership to build-out the health technology infrastructure that would allow a large community-based safety-net provider in Oregon to test and scale care delivery re-design and alternative payment models that incorporate new evidence-based practices to support change management, continuing education, skills development, quality improvement, and improved financial management. OCHIN will then provide shared learnings and be able to share economies of scale to other Oregon providers in order to expand the benefit of this investment. The investment would fund best-in-class technology infrastructure and technical support to implement a continuous learning environment, conduct research, and produce data driven best practices that advance alternative payment models that account for the clinical and social complexity of patients served by safety net providers and account for the substantial federal and state reporting requirements. The next generation infrastructure and tools will be implemented to inform population health management while integrating and streamlining care management and coordination, operations, administration, and public health functionality to ensure the success of the Medicaid payment reform programs and address current and future public health threats. This project will support improved, whole patient care for providers serving historically underserved patients utilizing next generation technology infrastructure and technical support to maintain systems. The investment would fund resources to address the specific needs of safety-net providers (including tailored specifications, workflow re-designs, intensive technical support, and ongoing informatics support).
