Fiscal Year 2023 Capital Assistance for Hurricane Response and Recovery Efforts (CARE) - Address: 8906 Two Notch Road, Columbia, SC 29223 Project Director Name: Nathan Prabhakar Contact Phone Numbers: (803) 764-0935 Email Address: nathanp@genesisfqhc.org Website: www.genesisfqhc.org Genesis Healthcare, Inc. (GHC) is requesting Capital Assistance for Hurricane Response and Recovery Efforts (CARE) funding to assist with the Genesis Healthcare Hurricane Response Mobile Site Project. GHC is submitting a one-time equipment-only funding request for assistance with the purchase of a mobile clinic site to increase its preparation for future hurricanes and other emergency disasters impacting the communities served by its clinics and pharmacies. Additionally, the mobile clinic site will allow GHC to increase its capacity to provide high quality healthcare to the underserved communities of South Carolina. The mobile clinic will assist with the organization's efforts to improve access to patient care, increase the health outcomes, and the ability to work as a backup when fixed sites are not operational. When the mobile unit is not supporting clinic sites impacted by disasters, it will be utilized for vaccination clinics, screening events, and other community events to promote healthcare to the underserved. The purchase and use of the mobile clinic site will result in continued care during disasters and increased availability of services for improving health outcomes and reducing healthcare disparities within the underserved communities of the Pee Dee and Lowcountry regions of the state of South Carolina. After receipt of the notice of award, GHC will submit a change-in-scope request for HRSA approval. Once the approved change-in-scope is received, the mobile clinic will be ordered. After delivery, inspection, payment, GHC will draw down the awarded funds. The projected date of project completion is May 2026. This will be a jointly funded project with a proposed total budget of $612,500, that will include funding from Genesis totaling $122,500 and once approved, a one-time CARE funding totaling $490,000.
