Fiscal Year 2023 Capital Assistance for Hurricane Response and Recovery Efforts (CARE) - Foothills Community Health Care, Inc. (FCHC) provides individuals and families in Pickens County and Anderson County of South Carolina, with quality primary care in a timely, efficient, and consistent manner through three fixed service delivery sites in Clemson, Easley, and Anderson. The organization was conceived in 2007 through a comprehensive planning process implemented by the South Carolina Primary Health Care Association in coordination with Pickens County community leaders. Funded as a federal New Access Point (NAP) in 2012, FCHC officially opened its doors to the community on April 30, 2013. Since that time, FCHC has worked diligently to establish the infrastructure, systems, and resources necessary to serve a high-need area that previously lacked any health center program. Proposed Project: On August 30th, 2019, Foothills Community Health Care (FCHC) won a New Access Point (NAP) to open its third site in Anderson, located at 1100 West Franklin Street. The site was previously occupied by AnMed Health, the local hospital group in Anderson, which served private insured adult primary care practice. FCHC converted this site to a Federally Qualified Health Center to serve the uninsured and underinsured along with continuing to serve the existing patients of this location previously occupied by AnMed Health. There continues to be an unmet need in and around Anderson County. The ZCTAs in this region of South Carolina are: 29624, 29684, 29655, and 29626. According to HRSA UDS Mapper, the service area has a total population of 40,505. Of these, 20,101 individuals (49.6%) live with incomes below 200% of the Federal Poverty Guideline (FPG). Within Anderson County, the South Anderson City and Belton Division are designated as Medically Underserved Areas (MUA) (HRSA MUA Find). Only 2.7% of residents in the proposed service area are served by the health center program (HRSA UDS Mapper). The current location of FCHC in Anderson is located inside a 5,682 square foot section of 1100 W. Franklin Street which was previously the Anderson High School Building. In this location FCHC was only able to create 6 exam rooms, 1 Counselors Office, Lab Room, Waiting Room and an onsite 340B Pharmacy. Foothills Community Health Care’s proposed Capital project will purchase and renovate a 29,000 SQFT building on a new site just 2 miles from the existing location. Once completed FCHC would relocate the exiting health center site to this new location. The new building will provide more examination rooms, a waiting area, records rooms, patient services, provider offices, a special purpose room, dentistry office, pharmacy, restrooms, healthcare support function areas, communications, and mechanical spaces. The building will comply with all accessibility requirements of the Americans with Disabilities Act and local building codes. An FCHC sign will be installed at the entrance of the facility, and moveable medical and office equipment will be purchased. The resulting facility will result in a larger and better organized facility, well-suited to deliver services to the underserved patient population as well as allow Foothills Community Helath Care to respond to future emergencies. As an existing health center with an established history of strategic expansion to meet the healthcare needs of the low-income and uninsured, FCHC is well-positioned to continue to address unmet need in this region.
