Fiscal Year 2023 Capital Assistance for Hurricane Response and Recovery Efforts (CARE) - Eau Claire Cooperative Health Center, Inc. dba Cooperative Health (CH) is one of South Carolina’s largest Federally Qualified Health Center (FQHC) serving low-income, medically underserved, and other vulnerable populations. In 2021, CH provided the full-continuum of comprehensive primary and preventative care services to include integrated behavioral health, dental, vision, women’s health, diagnostic, podiatry, and pharmacy to over 56,000 unique individuals (21% uninsured & 70% at/below 100% of the FPL). This resulted in 218,065 clinical visits in rural and urban geographies from 33 sites (permanent, seasonal and mobile) spanning four counties in the Midlands of SC (2021 UDS). CH also provides hospitalization, specialty referrals, and support/enabling services for all life cycles regardless of ability to pay. CH seeks to successfully compete for FY2023 Capital Assistance for Hurricane Response and Recovery Efforts (CARE) funding to alter/renovate (A/R) existing facilities without increasing the square footage of the facilities. Specifically, funding will be utilized to replace and repair the roof’s (exterior envelope) of the following: • Harden Street Medical Arts Center is a multi-disciplinary practice setting and is located in a Medically Underserved Area and federal empowerment zone. Three sites within CH’s scope of project are located under one roof and has an onsite 340b pharmacy: o Five Points Walk-In Center opened in 2007 and is accessible to all of CH’s pediatric and adult patients and has weekend hours. o Waverly Family Practice is co-located with Cooperative Health’s Ryan White Program (Parts C Early Intervention Services and Part D Coordinated Services for Women, Infants, Children and Youth). o Five Points Pediatrics provides full service to pediatric patients from infants, youth adolescent and young adults. Behavioral health (counseling and wellness) is integrated into the workflow. • James E. Clyburn Campus is a multi-disciplinary practice setting. The Clyburn Campus is located directly adjacent to a federally designated Empowerment Zone and part of Columbia’s low-income population Health Professional Shortage Area. CH has a 340b pharmacy located on the Clyburn campus. Along with the pharmacy, the following are housed in the same building (under one roof) on the Clyburn Campus. o Sterling Sharpe Pediatric Center opened in 1995 on Monticello Road, Columbia, SC and was relocated to the James E. Clyburn Health Center facility built on the same site and completed in the spring of 2010 with federal stimulus finds through the Capacity Improvement Grant. The pediatric practice occupies 7,000 square feet of the 11,000 square foot facility. o Eau Claire Internal Medicine on Monticello Road, Columbia, SC, was also relocated to the James E. Clyburn Health Center in the spring of 2010. It occupies a suite of 3,500 square feet. It provides comprehensive primary care to adults. • Brookland-Cayce Medical Practice is located in Cayce, a low-density urban area adjacent to Columbia’s HPSA/MUA/Empowerment Zone. The practice is fully dedicated to family practice, internal medicine and part-time obstetrics and gynecology. CH seeks total funding of $490,000.
