Fiscal Year 2023 Capital Assistance for Hurricane Response and Recovery Efforts (CARE) - Surry County is located in the Northwestern section of North Carolina. The population of the County is approximately 72,000 and has a poverty level of 16% and over 15% of the county residents are uninsured. When we assumed a pediatric practice in Surry County in the town of Elkin, Surry became the 85th of the 100 counties in North Carolina to have an FQHC presence. This site is currently located in a building that we purchased and renovated in 2019. The demand for services and the number of patients seen continues to grow there. Over 10% of the patients seen at this site are Spanish-speakers. In serving the pediatric patients in Surry County, we have witnessed the lack of available services for these children’s parents and other adults there. We have been seeking out a site to bring all of these needed services along with our pediatric services all in one location. After a great deal of research and collaboration in the community, we have located the perfect site. This location will allow us to locate all of the following services in one 18,000plus square foot building: • Adult Primary Care • Adult and Pediatric Behavioral Health Services • MAT/MOUD Services • Hepatitis C Clinic • Pediatric Services • Dental services for adults and children • 340B Pharmacy • X-Ray and Ultrasound The building in which we currently have the pediatric practice was originally built in 1985 and has a condo agreement with Tractor Supply which is adjacent to it. There are several difficult issues that would not allow us to expand at this site to include parking, plumbing, safety and others. The building with which we are currently under contract to lease and then purchase at 110 Dutchman Court, will allow us to comfortably expand and have none of the aforementioned difficult issues. Because the physician we are buying the site from is down-sizing, we are selling our current site to him. We will be able to purchase the new site through a USDA loan at a price that is over $1million below the appraised value and benefit from much of the equipment and supplies he is leaving behind. Having this high quality and more modern space and providing these additional services, will support the continuity of access to high quality primary care services for underserved populations that have not previously existed in this county. When disaster hits and when services are needed, HCCH will be there to provide comprehensive services to this North Carolina region. These services will be provide to all including the underserved population. These professionals will ensure that our walls, doors, windows, rooms, and openings are designed to withstand high winds and water pressure, therefore protecting our staff, services, systems, and structure. We plan to use this funding to help renovate and up fit the portion of the new site that will be used for much needed pediatric and adult dental services in Surry County. HCCH looks to open an an in-house pharmacy on the first floor. Security cameras will be installed as well as roll down locking doors. Our project timeline is to have the sale completed by September 2023. In anticipation of that final sale date, we have already begun working with our Project Manager, 330 Partners, and our Architects, LS3P on schematics for the renovations. We plan on starting immediately after the building purchase on getting our pediatric practice up and running while we renovate for our dental practice upstairs and our pharmacy, our goal is in 2 weeks’ time. Our target opening for the new dental facility is January 2024. HCCH will continue to upgrade this new building in order to add more services.
