Fiscal Year 2023 Capital Assistance for Hurricane Response and Recovery Efforts (CARE) - Compassion Health Care, Inc. (CHC) proposes using CARE funds to undertake two (2) projects: Project 01: Construction/Expansion (C/E) of an Existing Facility C/E will take place at: Caswell Family Medical Center 439 US Highway 158 Yanceyville, NC 27379 CHC will add an automatic transfer switch for the generator, and expand the parking lot at its Caswell Family Medical Center location. The added space in the parking lot will be used to park the new mobile health unit when not in use. Project 02: Equipment Only CHC will add a mobile health unit. The mobile health unit will be added as a site, will be parked at 439 US Highway 158, Yanceyville NC 27379 (Caswell Family Medical Center, Compassion’s main site). See Form 5B. The new mobile health unit will provide primary care services to school students in Caswell County. The mobile unit will be added as a site on Form 5B in accordance with CIS procedures, and will be parked at the main site at Caswell Family Health Center when not in use. The addition of a mobile unit will allow CHC to serve an additional 1,000 patients. This mobile unit will also be put into use following extreme weather events or emergencies to enable rapid access to services. The mobile health unit has an onboard generator, allowing it to operate during power outages.
