Fiscal Year 2023 Capital Assistance for Hurricane Response and Recovery Efforts (CARE) - Agape Community Health Center, Inc. dba Agape Family Health (AFH) is a 501(c)3 healthcare organization, headquartered in Jacksonville, Florida. AFH operates five service delivery sites throughout the Jacksonville metropolitan area. AFH is proposing to construction/Expansion (C/E) of an existing and new suite: full- time, permanent health clinic located at 1680 Dunn Avenue unit 34, Jacksonville, FL 32218. The service area of the proposed site captures eight zip codes: 32218, 32219, 32217, 32208, 32209, 32218, 32219, and 32217. This area harbors 230,745 residents, 42.8% of whom live at or below 200% of Federal Poverty Guidelines (FPG), establishing a target population of approximately 98,730 low- income residents who are made up primarily of recipients of public insurances and the underinsured/uninsured. This area describes southwest Jacksonville. Services at this new site will be available to all individuals. Primary medical care will be served by AFH directly at this site. In 2023, AFH served 11,104 individuals, 51.1% of whom received Medicaid, with an additional 1.2% under Medicare, and 47.7% were medically uninsured. AFH anticipates a similar insurance payor mix for its new site, thereby substantiating AFH’s role as a safety net provider to vulnerable, underserved, and underinsured populations of Jacksonville. In addition, in 2023, 89.1% of AFH’s patients were low-income, and 58.7% lived in poverty. AFH plans increase the capacity and capability to prepare for emergencies and/or disasters that will support the continuity of access to high quality primary care wrap around services for the underserved populations. AFH, wraparound service delivery will provide a comprehensive, holistic, adult and family-driven way of responding to prevention through health education, screening services, and maternal and child health. AFH proposed site will offer the follow services directly: Sexual Transmitted Disease (STD), HIV screening and testing, after hours, Maternal and Child Health, health educations services including nutrition with food demonstrations, chronic disease management, postpartum education, case management, outreach, and pharmaceutical education. These services put the family at the health center with support from a team of professionals and natural supports, the family’s ideas, and perspectives about what they need and what will be helpful drive all the work in wraparound especially during the time of a natural disaster. Wraparound services are what families need during times of uncertainty to received support for items and/or homes lost in the event of an emergency. AFH is proposing to purchase two generators to ensure that AFH health centers maintain appropriate power under all circumstances to keep patients and staff members and continued access to care. The generator will be purchased and install at three of AFH service delivery sites: 1680 Dunn Avenue suite 36-37, the proposed new suite 34, and 3900 University Blvd South, Jacksonville Florida, 32216. The generators will allow power to service delivery sites including immunization, vaccines, and medication refrigerators up to two weeks. AFH seeks Capital Assistance for Hurricane Response and Recovery Efforts funding to support the delivery of vital services to the underserved and underinsured of North Jacksonville. By end of the three-year project period, AFH will have served a minimum of 800 unique patients and 2000 unique visits.
