Fiscal Year 2023 Capital Assistance for Hurricane Response and Recovery Efforts (CARE) - The Community Health Foundation of Puerto Rico (CHFPR) Proposed Projects support alteration and renovation improvement costs necessary to meet the needs of the two clinic sites that were affected by both Hurricanes Fiona and Ian. As a vital part of PR’s safety-net health care services, CHFPR will address the needs identified to respond and recover from the previous hurricanes and to enhance the organization’s capability to prepare for, respond to, and recover quickly from future natural disasters to support the continuity of access to essential primary health care services for the underserved communities these health centers serve. CHFPR will complete “Alteration/Renovation (A/R) of an Existing Facility” to renovate two existing facilities without increasing square footage located at: • Project #1: Clínica Response & Recovery A/R – BPS-H80-029470: Community Health Foundation of Puerto Rico, Inc., Marginal Santa Cruz C-17 Urbanizacion Santa Cruz, Bayamon, PR 00961 • Project #2: Anexo Response & Recovery A/R – BPS-H80-032490: Community Health Foundation of PR, Inc., Carr 2 Marginal Santa Cruz, Esquina Esteban Padilla, Bayamon, PR 00961 Project #1: Clínica Response & Recovery A/R – The Clinica Project will modernize and improve the clinic site to address the current recovery needs and future sustainability including the following: • Reasphault and stripe the Patient Parking Lot Located in the Front of the Building and Staff Parking in the Back of the Building. • Paint Exerior of the Clinic. • Expand Exterior Lighting for Staff Parking Lot. • Replace Water-Damaged Acoustic Ceiling with Sheet Rock. • Update Lighting throughout Clinic with 2x4 flat panel LED interior lighting. • Paint Interior walls throughout the Clinic. • Replace Vinyl Flooring throughout the Clinic. • Update the Eight Clinic Bathrooms with New Vanities, Toilets, and Fixtures. Project #2: Anexo Response & Recovery A/R – The Anexo Project will also modernize and improve the clinic site to address the current recovery needs and future sustainability including the following: • Reasphault and stripe Patient Parking Lot Located in the Front of the Building. • Landscaping and Electric Gate for patient parking lot. • Exterior Safety Lighting for Entry and Parking Lot. • Paint Exterior of the Building. • Replace Water-Damaged Acoustic Ceiling with Sheet Rock. • Paint Interior walls throughout the Building. • Replace Vinyl Flooring through the Building. • Update the Two Existing Bathrooms with New Vanities, Toilets, and Fixtures. • Construction of one Additional Restroom for patients located in the front of the building. • Purchase and Install Power Generator.
