Fiscal Year 2023 Capital Assistance for Hurricane Response and Recovery Efforts (CARE) - Throughout the years, Camuy Health Services, Inc. (CHSI) has been preparing to continue to be recognized as an organization that offers comprehensive and preventive health services on an ongoing basis. This takes into consideration, the experiences lived through the 36 years of existence of the organization, the requirements of Appendix Z of CMS and our Continuity of Operations Plan. Regarding our recent experiences, lived with Hurricanes Irma, Maria and Fiona are notable. These had a significant impact on Puerto Rico, as it tested the measures planned for the management of emergencies at the level of government agencies, as well as private entities. The lessons learned, were many and led us to conclude that our emergency management plans had to be reviewed and updated frequently. Based on the aforementioned, CHSI has decided to submit our request for Fiscal Year 2023 Capital Assistance for Hurricane Response and Recovery Efforts (CARE) for two projects. Both projects will use the total of $490,000 which is the amount of funds available under CARE. Both projects will be carried out at our main site in which CHSI owns the property. Additionally, these projects are classified as Alteration/Renovation (A/R) of our existing facility, which is our primary site. They do not increase the square footage of the facility and will be held at our main site which is within our current service area. CHSI applied for public assistance from FEMA in category B under the disaster declaration registered as Puerto Rico Hurricane Fiona (DR-4671-PR). One of the projects is titled Expanding the Umbrella. It is aimed at renewing the sealing of the roof of our facility. The original sealing was done in 2013 and due to inclement weather, including the passage of Hurricanes Irma, Maria and Fiona through Puerto Rico, have affected the durability and efficiency of the material used to seal the roof, creating many leaks. These leaks cause interruptions in the continuity of operations, limiting patient access to the services we offer. In the last two years, several incidents related to roof leaks have arisen, which have caused interruptions of the services or operations at the department where the leaks occur. On occasions, we have been able to mobilize staff and patients to another location but on most occasions, operations are interrupted. This project has an estimated total cost of $272,160 which includes $242,200 at an average unit cost of $8.65 per square foot, with a roof of approximately 28,000 square feet (Space to Renovate), $9,800 for removal and disposal of existing material plus $20,160 for fees project inspection. Another project to be financed with CARE Funds is titled Strengthening the foundation of my Solar Power System. It is aimed at renovating the structure in which the solar panels are installed to provide greater protection from the winds of Hurricanes and/or Storms. The current structure was installed in 2013 and it is deteriorated by corrosion that covers more than 30% of the structure. Ensuring that the foundations, where the solar panels are located, provide us with the reassurance that our ability to generate solar energy is not affected due to the structure being destroyed by the winds caused by a Hurricane or Storm. This will allow us to have continuity with our Solar Power System and at the same time provide the necessary energy for our Tesla battery bank that is connected to the critical areas of the facility. This project has an estimated total cost of $217,840 which includes $125,500 for the construction and installation of the new steel structure, for a roof of 4,000 square feet, $40,500 for removal and disposal, $25,000 for epoxy paint, $15,000 for equipment rental, plus $11,840 for project design and inspectio
