Fiscal Year 2023 Capital Assistance for Hurricane Response and Recovery Efforts (CARE) - Since 1969, Rocky Mount Opportunities Industrialization Center (OIC) has provided comprehensive employment training, business and health services that strengthen opportunities for people in communities we serve. As the only FQHC in Rocky Mount, we are committed to full life cycle primary care and wellness, behavioral health, pharmaceutical services, and care coordination services to high-risk families. Our model of integrated health care services, provided through our medical sites and Mobile Area Health Clinic, has been proven over time to be indicative of OIC’s motto: "Helping People Help Themselves." Rocky Mount is the urban center of the Twin County Region. Most of OIC's patients live in neighborhoods with dilapidated & substandard housing, have genetic predispositions to diabetes, hypertension/allied diseases, obesity and have limited access to enrichment opportunities. We have a special interest in under-resourced minority youth, young adults, and seniors, as well as our identified cohorts of chronic disease patients and at-risk students. Opportunities Industrialization Center, Inc. is a one stop-shop for job training, human resources and healthcare that has lived in the hearts of Rocky Mount Residents for fifty years. Founded as a grassroots training hub, OIC has grown from providing education services to fulfilling the health needs of our students and their families. Our roots are grounded in evidence-based approaches to bridging the gap between social inequities and better health outcomes. There is a need to address the systemic issues and multi-generational traumas that are impacting the hearts, homes, and health of the Twin Counties. Rocky Mount is the urban center of the Twin County Region. Most of OIC’s patients live in neighborhoods with dilapidated and substandard housing. Many of our patients have genetic predispositions to diabetes, hypertension/allied diseases, obesity and have limited access to educational opportunities. Although our project benefits Upper Coastal Plain region residents, local industries, and the regional economy, OIC is targeting and recruiting residents located in Edgecombe County, primarily Zip Code Tabulation Areas (ZCTA) 27801 and 27802 for the ITA. The total population of the targeted ZCTA area is 20,953 (NC Hometown Locator) and we currently serve 3,050 patients in this area. Approximately 77% of our patients have incomes below 200% of federal poverty guidelines (FPG) and over 52% of our patients served in CY 2021 identified themselves as being a minority and 30% refused to identify their race/ethnicity when they registered for services at our offices. (HRSA UDS Report 2021). During CY 2021 our agency served 16,451 patients who generated 75,220 visits. We have a special interest in minority, poor individuals over 18, as well as our identified cohorts of chronic disease patients, at-risk students and returning citizens. Funds will be utilized to improve the safety, security, upkeep and availability of our facilities and resources for the community residents that we serve. Funds will be used to 1) repair an elevator in one of our medical facilities; without this elevator patients, staff and other visitors with disabilities that prevent them from using stairs will not be able to access our Dental Office, 2) replace awnings that cover the primary doors entering one of our medical facilities, 3) replace an HVAC system at one of our medical offices, 4) provide backup electric generators at three (3) of our medical offices to ensure medications, vaccines and other critical clinical resources that require refrigeration are not damaged beyond use when our facilities are impacted by power outages and 5) paint interior and exterior of our FY 2023 CARE Project facilities.
