Fiscal Year 2023 Capital Assistance for Hurricane Response and Recovery Efforts (CARE) - Family Health Centers of Southwest Florida, Inc. (“Family Health Centers”) proposes to construct a new +/- 19,000 sq. ft. standalone building to provide medical, dental, and medical social services in Labelle, Hendry County, Florida. On September 28, 2022, at 3:05 pm the center of Category 4 Hurricane Ian made landfall at Cayo Costa. The 150 mph winds and catastrophic storm surge severely battered Lee County, Florida, for hours. Nine months post-Hurricane Ian, Southwest Florida continues its recovery and restoration efforts. This new building will increase capacity and capability to prepare for, respond to and recover form future emergencies and disasters that will support the continuity of access to high quality primary care services for underserved communities. The new Labelle Clinic will be built on land owned, mortgage-free, by Family Health Centers. The 19,000 square foot building will house a pediatrics clinic, dental clinic and Social Services offices. Each department will have provider offices, triage rooms, exam rooms, Medical/Dental Assistant areas, waiting rooms, toilets, reception, administrative support, and IT space. Family Health Centers will move the staff, equipment, and supplies currently at the Labelle Medical & Dental Office (BPS-H80-002195) to the new clinic upon completion. Each clinic will buy any additional equipment with other funding on an as-needed basis. The new structure will be larger than the current Labelle service site. The new larger facility will supply added clinical space to accommodate more patients and provide patient services in a modern environment. In addition to offering pediatric, and dental services, the increased space will house adult medical services, on-site chronic disease management services and expanded social services, including the migrant outreach project.
