The purpose of the RBHC ZSI is “to improve the system of care for those at risk for suicide by improving the implementation of a comprehensive, culturally informed, multi-setting approach to suicide prevention on the Rocky Boy’s Indian Reservation.” To achieve this goal, we have developed the following measurable objectives, which line up with the goals of the Zero Suicide model:
Objective 1: Lead: The project will create a leadership-driven, safety-oriented culture committed to dramatically reducing suicide among people under care. This will be done through the development of a local Leadership Team.
Objective 2: Train: The RBHC ZSI will work to develop a competent, confident, and caring workforce. This will be done by training the staff on the Zero Suicide philosophy and the Health Center’s expectations of staff. Training will include educating all staff about suicide risk and training clinical staff on research informed practices for suicide care and treatment.
Objective 3: Identify: The RBHC will systematically identify and assess suicide risk among people receiving care. Currently, the RBHC uses PHQ-9 for adults and CDI-2 to screen children.
Objective 4: Engage: The RBHC ZSI will address the next element by ensuring every person has a suicide care management plan, or pathway to care, that is both timely and adequate to meet his or her needs.
Objective 5: Treat: This project will use ASIST evidence-based program that will directly target suicidality. ASIST addresses all seven elements of the Zero Suicide Model. The ASIST model involves assessment of suicide risk and development of a safeplan, which provides various options depending on present and future risk, available resources, and the needs of the person at risk.
Objective 6: Transition: At the transition stage of the project, we will provide continuous contact and support, especially after acute care.
Objective 7: Improve: Finally, the RBHC ZSI will apply a data driven quality improvement approach to inform system changes that will lead to improved patient outcomes and better care for those at risk of suicide. This element of the Zero Suicide Initiative will be ongoing. .
Ongoing assessment, inter-professional cross-training, and community training and outreach will increase system capacity to effectively achieve increased levels of integrated healthcare. This initiative will result in an integrated healthcare model which will provide patient-centered care for patients receiving services at the RBHC.
