Indian Health Board, Inc. (IHB) formally implemented integrated behavioral health four years ago when it was awarded the initial Behavioral Health Integration Initiative (BH2I) grant. With the support of IHB’s leadership including the board of directors, CEO and medical director, the integrated behavioral health team is fully imbedded in the primary care clinic and is located in the medical clinic. With participation and technical assistance from the BH2I grant initiative, IHB has progressed from level three to level five based on SAMHSA-HRSA Standard Framework for Levels of Integrated Healthcare. IHB has made significant strides in integration and plans to advance to level 6 “Full Collaboration in a Transformed/Merged Integrated Practice.” Our plan to advance to level 6 is by implementing the Patient-centered Medical Home, which we are already certified in, but due to changes in our electronic medical records system we have been unable to do certain aspects of this model and plan to implement the model more systematically as part of BH2I. There are five board goals proposed on in this project, which include improving the external referral process and system for children needing specialty services outside of the primary care clinic, increasing prevention and intervention activities for Native youth with type two diabetes, decreasing the number of IHB patients hospitalized two or more times in a calendar year as compared to the previous year, increase of number of patients enrolled in the opioid use disorder Medication Assisted Treatment and improving staff satisfaction and decreasing staff burnout. Thus, we hope to address a wide variety of pressing issues affecting the Native community with our proposal from trauma informed culturally grounded perspective.
