Over nearly 50 years, the Midwest Regional Spinal Cord Injury (SCI) Care System has provided a comprehensive continuum of rehabilitation services from onset to community reintegration and enrolled more than 2,100 individuals in the National SCI Database. For the 2021-6 funding cycle, our goals are to: (1) provide state-of-the-art medical and rehabilitative services to maximize outcomes following SCI; (2) contribute to the SCI Model Systems assessment of long-term outcomes; (3) conduct a site-specific project that enhances adherence to SCI exercise guidelines using smartphone technology, ecological momentary assessment (EMA) monitoring, and e-coaching; (4) engage collaborators in a modular project that evaluates biomarkers of neurologic and functional recovery; and (5) disseminate research findings to stakeholders in an effective manner. Database objectives are to enroll at least 90 people annually into the national SCI database and meet or exceed follow-up expectations for 1,200 database participants. Site-specific project objectives are to obtain input from people with SCI on how to customize smartphone-based apps, conduct a usability study of custom apps and EMA monitoring, and conduct a proof-of-concept SMART design study to determine the effective adaptive interventions. The collaborative module’s objectives are to evaluate spasticity and brain-derived neurotrophic factor as biomarkers of neurologic and functional recovery during inpatient rehabilitation. Outcomes include increased knowledge regarding SCI rehabilitation, effectiveness of an exercise intervention, and biomarkers of neurologic recovery. Anticipated products include smartphone apps and exercise protocols that promote adherence to exercise guidelines, biomarkers of neurologic and functional recovery, presentations and publications reporting research findings, expanded Rehabilitation Measures Database instrument summaries, and SCI Stories podcasts distributed via the Model Systems Knowledge Translation Center.
