Hawaii SHIP providing information and assistance to Medicare beneficiaries statewide. - Executive Office on Aging (EOA) is committed to achieving the proposed goal, objectives, and outcomes based on the themes of Service Excellence, Capacity Building, Operational Efficiency, and Innovation. Goal: Achieve greater recognition as a reliable source for accurate information, assistance, and referrals for Medicare beneficiaries, their families and caregivers, persons with disabilities, and hard-to-reach populations. Objectives: Hawaii SHIP as a part of the statewide No Wrong Door system, will expand partnerships and collaborations to leverage resources and maximize impact in the communities to increase access to services and supports for target populations. EOA will continue to provide person-centered, unbiased, and confidential counseling, education, and enrollment assistance to help beneficiaries make informed healthcare decisions based on their individual needs/budget; promote outreach to increase visibility; recruit, train, and retain a diverse, effective, knowledgeable, and culturally-sensitive workforce; strengthen infrastructure by improving policies, procedures, and organization for efficiency and cost-effectiveness; incorporate quality improvements to enhance services and sustainability; and remain adaptable to successfully position Hawaii SHIP for future changes in the programmatic landscape to maintain continuity of services and growth. Outcomes: 5% increase in contacts (phone, fax, mail, email, web-based, or in-person); 100% workforce retention; and 10% increase in volunteer/in-kind counselors. Performance measured by 1) Beneficiary Contact Forms for information, counseling, comparisons, Part C/D enrollments, referrals, volunteering; 2) Group Outreach & Education Forms for fairs/presentations; 3) Media Outreach & Education Forms for marketing/reach; 4) inquiry/response tracking; and 5) partnerships with state and community-based agencies. Products: Counselor aides, trainings, live/virtual presentations, brochures, articles, newsletters, promotional tools and ads, flyers, posters, surveys, web-based tools, Medi-Medi initiative, and live call center project.
