NH State Health Insurance Assistance Program - The State of New Hampshire Department of Health and Human Services, Bureau of Aging and Adult Services (NH DHHS, BAAS) submits this five-year State Health Insurance Assistance Program (SHIP) plan for the NH SHIP project that includes specific strategies to expand, extend, and enhance the outreach and one-on-one assistance efforts to Medicare beneficiaries. New Hampshire’s Unit on Aging (AAA) is the BAAS, and the New Hampshire Aging and Disability Resource Centers (ADRCs), formerly known as ServiceLink, are where the State Health Insurance Assistance Program team is located in NH. The State will again contract with eight (8) community providers to administer the SHIP program at twelve (12) ADRC offices for the purpose of providing unbiased, person-centered, accurate and comprehensive Medicare counseling and enrollment assistance. Our goals are to provide service excellence and empower, educate, and assist Medicare-eligible individuals, their families, and caregivers through objective outreach, consistent messaging, and knowledgeable counselors to make informed health insurance decisions that optimize access to care and benefits. We will do this through targeted population contacts, expanding our volunteer program to increase capacity, increasing NH SHIP’s organizational health literacy, and leveraging partnerships with other entities such as the NH Care Path, local LGBTQ+ and other cultural organizations, thereby strengthening our capability to support the SHIP mission. Anticipated outcomes include increased statewide awareness of the SHIP program resulting in an increased number of individuals assisted. Expected products include marketing and outreach materials to enhance program visibility.
