Strengthening Efficient and Consistent Access & Training: Elevating the Rhode Island No Wrong Door System for Seamless Service Delivery - The RI Office of Healthy Aging (OHA) envisions an enhanced, person-centered NWD system so consumers and caregivers can make informed decisions about their LTSS options. OHA builds upon the existing transformation efforts of the State’s LTSS system. Demand is expected to increase as the number of Rhode Islanders (RI’ers) that may need LTSS is expected to grow by 26%, from approximately 67,200 in 2018 to 84,800 in 20301. RI’ers will need support in making decisions about their LTSS options. The focus of expanding outreach, education, and partnerships, to increase capacity and use of HCBS while decreasing institutional care utilization across the State. Individuals and caregivers will be educated about LTSS options and given the opportunity to be engaged in the decision-making process, acknowledging their options, values and preferences, and ensuring successful transitions to community living for nursing home residents. To achieve reform, it is critical to educate individuals and caregivers about LTSS. Under the direction of Governor Dan McKee, the leadership at OHA lives the vision to enhance the NWD system to promote the range of LTSS options for RI’ers and rebalance the LTSS delivery system. The OHA will enhance systems and operations to the "NWD" system in the State of Rhode Island and support consumers and caregivers in making informed decisions about their LTSS options while continuing to streamline access for individuals seeking assistance, across various health and human service organizations, and strengthening the ADRC. RI is committed to enhancing the efficiency of training staff in the NWD system. Targeted training will ensure NWD staff have the knowledge to improve access and utilization for all individuals and caregivers. developing a coordinated program that is efficient and consistent statewide. Goal I: Improve user access and service coordination to ensure appropriate support regardless of the initial point of entry. Objective 1: Increase community awareness of the "NWD" system to boost engagement and utilization. Objective 2: Ensure that care and services are person centered and conflict free, providing options, and safeguarding individuals desire to receive services in the manner of their choosing. Objective 3: Expand partner networks across RI to ensure integrated care and services utilizing multidisciplinary care teams. Outcome: Enhanced user experience with faster more efficient access to PCOC, helping individuals and caregivers, make educated decisions about the care they receive. Goal II: Develop ongoing support structure for data systems. Objective 1: Complete implementation of an integrated digital platform connecting all participating agencies and community partners enabling seamless HIPAA compliant information exchange. Objective 2: Complete user acceptance training and validation to train staff across all agencies on the updated system ensuring efficient and empathetic delivery of services. Outcome 1: Improved Collaboration and communication amongst agencies and community partners reducing service duplications and/or gaps in service. Outcome 2: Improved satisfaction and outcomes for individuals utilizing the system as measured by regular feedback, tracking of KPI’s and more comprehensive reporting capabilities. Goal III: Diversify funding to establish Medicaid administrative claims. Objective: Implementing MAC for NWD functions, to allow for eligible federal reimbursement. Outcome: Develop a sustainability plan using alternative revenue stream, and exploration of innovative funding mechanisms to ensure long-term financial stability for the NWD system. Goal IV: Create coordinated, efficient training for NWD LTSS counselors. Objective: Implement a comprehensive training program that equips NWD LTSS counselors with the knowledge to provide accurate, seamless, and effective service options. Outcome: RI’ers receive personalized support regardless of their point of entry.
