Falls Free Missouri: Scaling Up STEADI and Stepping On will expand screening tools and falls prevention programming statewide, and over three years will serve 6,360 older adults. - Project Summary: Goal 1: Oasis and statewide partners will scale up adoption of the STEADI screening tool in FQHCs, support social care referral products that connect clinics to community-based organizations, and enroll participants in Stepping On (SO). Goal 2: Oasis will document successes, challenges, and lessons learned across the grant period and disseminate approximately 15 Section 508-compliant resources statewide to inform practice, sustain evidence-based falls prevention programs, and support replication of this model. Objectives: (1) scale up STEADI adoption in FQHCs statewide, screening at least 2,340 participants; (2) scale up Stepping On statewide, enrolling at least 4,020 participants; (3) directly implement STEADI (serving 240 of 2,340 total) and SO (serving 1,000 of 4,020 total) in the St. Louis Metro area; (4) assess/support technology to streamline referrals and data interoperability between clinic and community partners; (5) manage the project to ensure all elements are delivered as expected; (6) align with other federal and statewide investments for long-term sustainability; and (7) ensure all products are Section 508-compliant. Approach: Oasis will coordinate with clinical partners on STEADI screenings, license SO statewide, coordinate leader training with the program's developer, and partner with health centers, AAAs, and healthcare systems to connect people who screen "not at risk" for falls into SO workshops, offered in person and virtually so rural residents can take part. Outcomes: at least 70% of participants (2,814) will complete the program, with gains in balance confidence, physical activity, and reduced isolation, and 22% experiencing fewer falls. Products: SO toolkit, semi-annual findings reports, referral workflow guide, recorded webinars, and outreach toolkit for ages 60-70. Duration: 9/30/2026 - 09/29/2029.
