In partnership with the State of Ohio, Kent State University is proposing a proof of concept for the goal of seamless transition planning for youth identified as having intellectual disabilities, multiple disabilities, traumatic brain injury, and autism who typically categorized as having cognitive disabilities in the VR coding system. The objectives of this project are to: 1. Selecting 25 VR-SPED teams and 125 target youth three years from exit in 4 regions of Ohio. 2. Engaging a Design Team for VR-SPED professional development/coaching in transition. 3. Provide professional development/summer follow up for the 25 teams and collaborators. 4. Provide teams coaching in IEP/IPE development, joint goal processes and improvement monitoring tool for team needs in Years 2-4. 5. Advising and consulting to support placement of 125 youth in competitive employment. 6. Maintaining and evaluating PDCM model processes. 7. Replicating process for 25 new VR/SPED teams and 125 target youth in Year 4. 8. Developing materials for dissemination to prepare for state/national utilization. Anticipated outcomes include: (a) joint and evidence-based IEP/IPE planning, (b) seamless transition services between school and postschool, and (c) 30-60% improved postschool outcomes for the participating cohort. The products of this project will include: (a) materials to develop joint professional development for VR counselors and special educators, (b) materials to coach VR-SPED teams in developing joint and evidence-based IEPs and IPEs, (c) dissemination materials for diverse transition stakeholders, and (d) proof of the efficacy the model in promoting improved employment outcomes for youth with cognitive disabilities.
