Sexual wellness, including ability to participate in sexual activities, quality of intimate relationships, and identity as a sexual being entitled to full societal participation, is a key dimension of recovery after traumatic injury. However, many individuals with spinal cord injury (SCI) or traumatic brain injury (TBI) experience a significant post-injury reduction in sexual wellness. Unfortunately, psychometrically rigorous outcome measures of sexual wellness after SCI or TBI and empirically supported intervention strategies are lacking. The goal of this five-year project is to create and evaluate a web-based symptom-monitoring/self-management system to improve sexual wellness in individuals with SCI or TBI. We will finalize psychometrically robust monitoring tools (i.e., patient-reported outcome measures) of sexual wellness for individuals with SCI or TBI. We will develop four item response theory-calibrated item banks that assess aspects of sexual wellness for individuals with TBI and four item banks for individuals with SCI; all eight item banks will use a web-based delivery system. We will work with individuals with SCI or TBI, their intimate partners, and SCI/TBI clinicians to develop clinically relevant cut point scores using a consensus-based technique. Finally, we will select evidence-based self-management intervention strategies through literature review and stakeholder feedback. We will develop intervention videos to treat and improve sexual wellness in individuals with TBI/SCI. The ultimate deliverable of this project will be an intervention platform (iManage) that will teach individuals with SCI/TBI to independently monitor and improve their sexual wellness by completing the new measures and watching intervention videos prompted by their scores. The ultimate outcome of this project is improving sexual wellness in individuals with SCI or TBI.
