Growing the Wellness Integrated Network of Washington to Deliver and Sustain Evidence Based Chronic Disease Self-Management Education and Support Programs for People with Behavioral Health Conditions. - Comagine Health and nine statewide partners will, over this 3-year project, develop a coordinated Wellness Integrated Network of Washington (WINWA) to expand CDSME/support programs to reach counties and older adults/people with disabilities with behavioral health (BH) conditions/stressors – depression/new chronic disease/housing instability.Programs- CDSMP/CPSMP/DSMP/ Enhance®Fitness (EF)/PEARLS - were selected for proven BH impact. Training/program delivery (in-person/virtual) begins within three months of award. Goals to 1) develop WINWA; 2) coordinate data systems; 3) increase program access/engagement/benefit; 4) sustain reimbursement; 5) develop/disseminate resources. Objectives are 1.1) establish steering committee; 1.2) complete business plan; 1.3) implement delivery – 68% counties by 8/27, 100% by 8/28; 2.1) onboard nine new Compass partners; 2.2) integrate ODES/WellWare; 2.3) develop BH referral; 3.1) enroll each 60 (42 completers) in CDSMP, 200 (140) in CPSMP/DSMP, 3,000/EF, 300/ PEARLS; 4.1) complete market analysis; and 5.1) develop 5 resources by Year 1, 10 by Year 2, 20 by Year 3. Outcomes include 1.1) workshops in 100% counties; 1.2) coordinated CDSME network/committee; 2.1) coordinated data system; 2.2) referral workflows; 2.3) three BH partnerships, 3.1) increased self-management and reduced isolation, 4.1) payer contract, 5.1) resource/toolkit. Key products Statewide business plan for WINWA sustainability, integrated data system with BH referral workflows, 20 508-compliance resources (marketing toolkit included), capstone project toolkit synthesizing partner best practices for replication.
