Chronic Disease Self-Management Education CDSME State Grants - The Sanford Center for Aging (SCA) at the University of Nevada, Reno, seeks funding to expand CDSME programs statewide. CDSME programs included were chosen based on relevance to prevalent conditions in Nevada and their overlap with behavioral health: Chronic Disease Self-Management (CDSMP), Diabetes Self-Management (DSMP), and Health Coaches for Hypertension Control (HCHC). The support programs are Fit & Strong!® (F&S) and EnhanceFitness (EF). The goal is to expand statewide capacity to deliver CDSME and support programs while promoting sustainability by developing resources for program implementation. Objectives for the proposed three year initiative are to: 1) Train 25 additional workshop leaders across seven counties; 2) Expand referral networks with behavioral health specialists; 3) Improve access by offering 148 total additional workshop series; 4) Increase caregiver engagement; 5) Ensure person-centered program delivery; 6) Identify and train peer leaders in rural counties; 8) Work towards diversified and sustainable funding; and 9) Develop and disseminate resources to increase knowledge of the field. Anticipated outcomes include improved chronic disease and exercise self-efficacy, improved mental well-being, increased awareness of CDSME programs among providers, and increased knowledge of implementation best practices. We will reach 212 completers in CDSME and 316 participants in support programs in year 1 and 251 completers and 316 participants both in years 2-3. We will reach seven Nevada counties by working with four key partners who operate across the state: UNR Extension, Dignity Health St. Rose, Lyon County Senior Services, and Nye Communities Coalition.
