Chronic Disease Self-Management Education CDSME State Grants - The goal of “EHRI CDSME Behavioral Health” is to (a) increase statewide capacity to provide chronic disease self-management education (CDSME) to Delaware’s senior and disabled adults with behavioral health challenges; and (b) develop CDSME resources for providers and the public which will be disseminated statewide and ensure program sustainability and replication. The project targets Delaware residents 60 years or older and disabled adults who suffer from behavioral health issues, including depression, anxiety and PTSD, or have specific acute behavioral health stressors, such as loss of a loved one, change in finances, or a new diagnosis of a chronic condition. To accomplish its goals, EHRI will build an expansive statewide network of behavioral health agencies and providers situated throughout Delaware to deliver and sustain the program during and after the grant period. Partners include behavioral health agencies Mental Health Association in Delaware (MHA), Jewish Family Services (JFS), LaRed (an FQHC with integrated behavioral health services at ll 3 service locations) and Beebe Healthcare; as well as Modern Maturity Center ( the sole Title III provider in central Delaware and northern Sussex County, with a robust cadre of volunteers who have served as CDSME lay leaders), Pace Your Life (a dayhab program), Easterseals (which services disabled and older adults and their caretakers) and Reliant Care Solutions (experts in safe and effective medication use). EHRI will recruit and train 40 new lay leaders in CDSMP delivery (to double the size of an existing network of 43 volunteers with which we will launch the program); develop statewide in-person and virtual program delivery options (locations, sites and formats); recruit program participants; and integrate CDSMP BH with its statewide WeCare nurse-based care navigation network to provide wraparound support services to CDSMP participants and help them sustain learning. EHRI will offer participants both the universally applicable ACL-approved evidence based CDSMP and HomeMeds programs. Measurable objectives include: (1) recruitment of 600 CDSMP participants, with a 70% completer rate, and 200 HomeMeds participants during the three year program; (2) recruitment and training of 40 new CDSMP lay leaders; (3) a minimum of 50 CDSME workshops delivered across the state; and (4) nine section 508 compliant educational resources capturing best practices, produced and disseminated statewide. The program will result in the following expected outcomes: (1) A substantial increase in the number of older adults with behavioral health conditions and/or stressors who participate in CDSMP and HomeMeds evidence ; (2) A doubling of the number of trained CDSMP lay leaders; (3) a robust statewide network of behavioral health agencies, healthcare, older adult and disabled adult providers created who promote CDSME for their clients with behavioral health issues; (4) a statewide infrastructure of providers who will promote CDSME as a critical health practice, especially for older and disabled adults with behavioral health challenges during and after the grant period; and (5) resources reflecting best practices disseminated to providers and the public via the program-created website and other venues.
