Pathways to Connection: A Continuum of Adoption Supportfor Children with Complex Needs - Children transitioning from foster care to adoption with complex behavioral, developmental, medical, mental health, trauma-related, and relational needs require specialized support that extends beyond adoption finalization. The University of Oklahoma, in partnership with Oklahoma Human Services (OKDHS), Oklahoma State University Center for Health Sciences (OSU-CHS), and community-based clinical partners, proposes Pathways to Connection: A Continuum of Adoption Support for Children with Complex Needs, a three-year project to expand, implement, and evaluate a coordinated statewide continuum of adoption-competent services. Building on approximately 2.5 years of formative implementation, Pathways integrates three interconnected components: (1) a specialized Foster Care and Adoption ECHO providing case-based consultation and adoption-competent expertise to clinicians and providers; (2) a structured, approximately 12-week adoption-transition clinical intervention providing assessment, psychoeducation, caregiver coaching, relational and family-systems support, and care coordination for families caring for children with complex needs; and (3) ongoing adoption-competent clinical care through the OKDHS Post Adoption Services Clinical Team or qualified community providers. A prospective mixed-methods implementation and effectiveness evaluation will examine implementation, reach, provider capacity, caregiver and family outcomes, child well-being, service continuity, and permanency-related outcomes. The project seeks to strengthen families and the systems surrounding them while creating a sustainable pathway to adoption-competent support from foster care through adoption and beyond. Findings will inform refinement, sustainability, and future replication of the model.
