Community Care Corps: Expansion and Refinement of Volunteer Assistance Models - Oasis, USAging, Caregiver Action Network and Altarum propose continuing their partnership to further enhance, expand and refine the Community Care Corps program developed and implemented over the past five years. In this proposal, we address Priority Area 1: Improving the nation’s overall capacity to support volunteer Community Care Corps programs and Priority Area 2: Advancing innovative local models that support volunteer care corps programs providing nonmedical care. The two priority areas will be achieved through the following project goal: Increase the number of community-based volunteer programs available to provide nonmedical care to older adults, persons with disabilities, and their caregivers to maintain independence at the local level while decreasing the number of older adults, persons with disabilities, and family caregivers who need assistance in maintaining independence at the local level but are unable to obtain help. Project objectives include 1) Establish an inclusive Advisory Committee to support and advise the leadership team; 2) Enhance and employ an unbiased grantee selection process; 3) Execute and manage selected grantee local models; 4) Support, collect and analyze data through use of preand post- surveys for participants and volunteers; 5) Implement a stakeholder engagement and communication management plan; 6) Work with grantees to create sustainability plans for continuation of local models after funding ends; 7) Collect, share, and submit relevant reporting. Outcomes include reduced caregiver stress, reduced loneliness, increased volunteer retention, increased program efficiencies, increased confidence in maintaining independence in the community, and increased utilization of person-centered approaches for volunteers and model participants. Products include presentations, press releases and expansion of the Effective Elements Learning Library.
