There are nearly 1 billion visits to office-based health care clinics every year in the United States, supported by a growing array of Health Information Technologies (HIT) intended to improve the quality and safety of care. The 2009 HITECH Act was instrumental in driving towards widespread EHR adoption such that by 2017, 86% of office-based physicians had adopted an EHR. HITECH and subsequent policies and regulations have also worked to advance health information exchange, most recently by requiring that certified EHRs include standards-based application programming interfaces (APIs). The Office of the National Coordinator for Health Information Technology (ONC) has not only played a key role in policy development and implementation, but also in measuring the adoption, use, and impact of HIT. A primary source of data is annual surveys of hospitals and office-based physicians on adoption, use, and barriers related to EHRs, interoperability, and other topics. In this project, the American Board of Family Medicine (ABFM) and the Center for Clinical Informatics and Improvement Research (CLIIR) at UCSF propose a partnership to meet ONC’s requirements for understanding of the implementation and effects of federal HIT policies as well as disparities or unintended consequences resulting from their implementation. These partners will co-develop and administer a survey that builds on unique existing and nationally-representative census-surveys of ABFM diplomates reflective of primary care outpatient settings, a critical source of insight given that nearly half of all people who seek care will do so only in primary care. Additionally, they will collect survey data from a nationally-representative sample of office-based subspecialists to ensure comparable data for primary care and specialist office-based physicians. Survey data will serve to track longitudinal measures of EHR adoption and interoperability engagement as well as add new, timely measures of API adoption, telemedicine use, and other topics. These data will also allow assessment of challenges and barriers, as well as differences by key physician and practice characteristics. The ABFM and CLIIR bring deep experience in survey-based assessments to the proposed partnership with ONC. Our prior work reveals important disparities in EHR adoption and functionality, particularly for small, rural, and independent practices. Both ABFM and CLIIR have learned from previous efforts about the difficulties outpatient clinicians face in easily sharing patient data between settings when it matters most, and when practices decide to change their EHR (interoperability). In addition, recent studies show that EHRs contribute to burnout in our health care workforce. Over the next three years, we aim to more systematically collect physician feedback with the goal of improving functionality while reducing burden. This partnership will publish and broadly disseminate its findings with the goal of informing the public, patients, and clinician organizations who work with federal agencies to improve care and clinical experience.
