Expanding innovative Dementia care support statewide by applying UCLA/VADC model, Skills2Care and other evidence informed practices directed to PLWD and caregivers in their home and in HCBS - VT’s goal is to build a sustainable dementia-capable Home and Community-Based Services system that expands the Vermont Alzheimer’s and Dementia Care (VADC) program based on the UCLA Dementia Care Model at a time when timely diagnosis and care is critical. ADPI funds will enable VADC to expand planning for its system of dementia-capable Hubs, Hublets and Spokes, as part of a No Wrong Door approach, taking Vermont’s effective model for opioid treatment and applying to dementia. Starting 9/26 through 9/29 with the state’s Dementia Services Coordinator as director, ADPI funds will: (1) expand dementia-capable HCBS access; (2) build capacity for timely diagnosis and management; (3) create new Dementia Care Specialists and Assistants; (4) deliver EBI training and caregiver services; and (5) conduct a robust evaluation and disseminate results as a long-term commitment to this work. Over 3 years funded by a $917,700 grant with a $331,796 match, VADC will expand primary care capacity through dementia-capable Hubs, Hublets and Spokes, including UVM Memory Program, Full Circle Medicine, Gifford Medical Center, Adult Days and FQHCs and deliver evidence-based provider and caregiver training, engagement and interventions through the Dementia Collaborative, Dementia Family Caregiver Center and Project ECHOs. Outcomes include increases in dementia care workforce, screening and diagnosis, clinical and community referrals, improving quality of life, reducing emergency room visits and hospitalizations. Dementia care wait times will fall and more caregivers will be supported by EBI interventions. Evaluation will perform required annual assessments, quality improvement, use state and partner data to assess outcomes and impact, and publish reports on VADC effectiveness and payment model for dementia HCBS.
