Nevada Dementia-Capable Communities (NDCC) - Nevada Dementia-Capable Communities (NDCC) Project Project Abstract Summary Neighbor Network of Northern Nevada (N4) will implement the Nevada Dementia-Capable Communities (NDCC) Project to expand person-centered, dementia-capable home- and community-based services for people living with dementia, people living alone with dementia, people with intellectual and developmental disabilities (IDD) and dementia or possible dementia, and their primary caregivers across Northern Nevada. The project goal is to strengthen N4's sustainable community-based service model so people can remain safe, connected, and supported in their homes and communities. This project will address five objectives: project management; supportive services for people living alone with dementia and their primary caregivers; supportive services for people with IDD and dementia and their primary caregivers; behavioral symptom management; and respite care and dementia-specific case management. N4 will provide dementia-specific case management, flexible respite care, evidence-informed IDEA! caregiver education and 1:1 coaching, IDD/dementia training with Positive Behavior Support-informed consultation, safety planning, service navigation, outreach, and evaluation. Key partners include the University of Nevada, Reno, Sanford Center for Aging (SCA) as third-party evaluator, State of Nevada Aging & Disability Services Division, Alzheimer's Los Angeles for IDEA!, Flinders University for Positive Behavior Support consultation, and Splaine Consulting for living-alone strategy. The NDCC project benefits from N4’s established working relationship with SCA and Splaine Consulting. Expected outcomes include improved quality of life for people living with dementia, reduced primary caregiver burden, increased service utilization with special target with who are living alone with dementia, improved IDD/dementia knowledge, and stronger behavioral symptom management. Products will include support plans, training and implementation tools, outreach resources, evaluation reports, dissemination products, and sustainability tools. The project period is September 30, 2026 through September 29, 2029.
