Expand identification of community-dwelling older adults in Chicago who are at risk or affected by ADRD (including those with IDD) to connect them and their caregivers with services - The Chinese American Service League (CASL) proposes a three-year project to address the growing Alzheimer's Disease and Related Dementia (ADRD) crisis in Illinois. Cook County ranks second nationally in ADRD prevalence, driven by high population density and pollution. With over 250,000 Illinois seniors living with dementia and nearly 500,000 unpaid caregivers facing significant health and financial strain, there is a critical need for scaling of dementia capable community-based support. CASL will leverage its established Brain Health program and Dementia Friendly America leadership to expand early detection and intervention for populations with limited access to care, including individuals with intellectual and developmental disabilities (IDD) and those living alone. The project will serve 1,500 individuals through enhanced partnerships, workforce training, and two evidence-based interventions— Support Health Activities Resources Education (SHARE) and the CARE Ecosystem—which provide person-centered behavioral management, care coordination, expert consultation, and caregiver support. Key strategies include implementing a new universal intake system to expand screening; partnering with community organizations (e.g., Alzheimer's Association, Chicago Public Library, National Task Group on Intellectual Disabilities and Dementia) for education; training 440 staff annually; and sustaining services through Medicare/Medicaid billing and the Centers for Medicare & Medicaid Services (CMS) Guiding an Improved Dementia Experience (GUIDE) Model. CASL brings a strong track record from prior ACL grants, having trained over 90 professionals, screened 614 older adults, and served 57 dyads. With a $7.1 million reserve and experienced leadership, CASL is well-positioned to improve quality of life for people living with ADRD, reduce caregiver burden, and build a dementia-capable community system in Chicago through this grant from October 2026 to September 2029.
