Dementia CARES will equip Community Health Workers to provide dementia screening, care navigation, and support groups for individuals identified in the early stages of dementia and their caregivers. - The County of San Diego Health and Human Services Agency (HHSA), Aging[JJ2.1] & Independence Services (AIS) will launch Dementia Community Awareness, Resources & Empowerment Support (Dementia CARES) to improve early detection, expand awareness of dementia‑related services, and strengthen support for people living with dementia (PLWD) and caregivers. Through Community Health Workers (CHWs), the program will provide validated screening, personalized care navigation, and outreach for individuals in early-stage dementia and caregivers. Early intervention will help identify needs sooner and reduce delays in accessing services. The project will integrate the evidence‑based Memory Club to offer a supportive environment for care dyads. Screened participants will receive referrals to brain health resources, including local exercise programs, engagement opportunities, and wellness supports. Dementia CARES will operate throughout San Diego County, focusing on individuals at risk for dementia or in earlier stages, with targeted outreach in communities experiencing poorer health outcomes. AIS will strengthen referral pathways by partnering with healthcare providers, academic institutions, and community‑based organizations, creating trusted access points. The program will also leverage existing Dementia Skills & Resource Fairs, which provide caregivers with hands‑on learning opportunities and connections to supportive services. By improving early detection and referral coordination, Dementia CARES will increase participation in the region’s dementia and caregiving support network and enhance quality of life for individuals and families. The program will use existing CHW tools, brain health outreach materials, screening and navigation resources, and referral workflows. Progress will be monitored through robust evaluation measuring early detection, service engagement, and wellbeing outcomes. The project will run from September 2026 through September 2029, with screenings beginning November 2026.
