A-CABS: Advancing Cognitive and Brain-health Supports - The goal of Georgia’s Advancing Cognition And Brain-health Supports (A-CABS) project is to achieve a dementia capable HCBS system that meets the unique needs of people living with Mild Cognitive Impairment (MCI) or early-stage dementia and their care partners. A-CABS objectives include 1) creating, expanding, and sustaining a dementia-capable HCBS that includes no wrong door access for people diagnosed with early-state dementia and their family caregivers; and 2) ensuring that Georgia’s HCBS systems provide innovative and evidence-based services to people with early-stage dementia and their families. Expected outcomes include AAA staff trained on early-stage dementia, an increase in the number of Georgians served by AAAs after being referred for services by Georgia Memory Net (GMN), a reduction in patient referral refusals, and an increase in patients accessing applicable AAA services. A-CABS will pilot an innovative, evidence-informed service, Cognitive Empowerment Program (CEP) that focuses on mild cognitive impairment/early-stage dementia. A-CABS addresses a gap in services for Georgia’s existing home & community-based services. The products produced will include a framework and script for AAA ADRC counselors to use when individuals with cognitive change seek Information & Referral. The framework will highlight the needs of people living with MCI/early-stage dementia, enhance workflows for referral and QI/QA, include on-going training for staff on MCI/early-stage dementia, and implement an evidence-informed intervention that addresses a gap in Georgia’s HCBS network. This project builds upon existing organizational and community-based capacity to promote sustainable public health interventions surrounding HCBS systems; and enhances Georgia’s collaborative and innovative network of aging services, clinicians, practitioners, advocates, and community leaders to affect lasting change in Georgia.
