The Utah Department of Health and Human Services (DHHS) intends to use funds from this opportunity for research and planning activities related to the development of a section 1945A State Plan Amendment (SPA) targeting Medicaid eligible children with medically complex conditions that will direct the implementation of a sustainable health home model for this population.
The primary objective of this project is to determine the feasibility and sustainability of a health home model for the target population in the state of Utah. The following goals have been identified as necessary to achieve this:

¿ Assessment of program design
¿ Evaluation of cost
¿ Evaluation of policy impact
¿ Identification of clearly defined outcomes
The bulk of the funds will be used to support the following positions: a program manager, a data scientist, and a grant manager. The program manager will be needed to ensure progress of the project, adherence to federal guidelines, and to facilitate collaboration between involved stakeholders. This individual will oversee the day-to-day activities of the project and will direct and engage in the bulk of the work. The data scientist will be needed to gather, organize, and interpret data. The grant manager will oversee the overall grant project, ensuring that federal guidelines are followed, funding is used appropriately, and required reports are submitted.
Funds will be used for research and planning of the following items: possible system changes to allow health data exchange as well as notifications of access to care, selection of geographic location, recruitment and training of personnel, process/policies for (i) identification and education of eligible beneficiaries, (ii) granting access to and collaborating with out-of-state providers, (iii) monitoring established provider/health home standards and quality measures (iv) collecting/reviewing/disseminating reporting requirements.
