Abstract The Community Health Center, Inc (CHCI) is a Federally Qualified Health Center that provides primary care to over 150,000 low income patients throughout Connecticut. CHCI plans to increase the effectiveness of renewal support to existing patients and increase school and community outreach to increase new enrollment. CHCI expects to support renewals for over 5,000 children, 200 pregnant women, and 1,500 parents annually. Over 1,800 new enrollments will also be achieved. To accomplish this, CHCI hire an additional 4 Community Health Workers, launch a new bi-directional text-based outreach platform, increase school based partnership for outreach, and improve outreach eligibility an application support data tracking and workflows to better measure the impact of strategies and engage in continuous quality improvement. The total budget requested is $1,500,000 and the bulk of the funds will be used to hire additional staff and procure texting system licenses.
