Connecting Kids to Coverage HEALTHY KIDS 2022 Outreach and Enrollment Cooperative - Project Abstract
The Utah Health Policy Project (UHPP), a 501(c)(3) nonprofit, is the lead applicant for the Department of Health and Human Services Center for Medicare and Medicaid Services Connecting Kids to Coverage funding opportunity CMS-2Y2-22-001.
Utah has the highest rate of eligible but uninsured children in the nation, with only 79.4% of Utah’s eligible children being enrolled. Utah currently ranks 46 out of 50 states for covering kids.
UHPP is well-positioned to address these needs in the state of Utah. For the past nine years, UHPP led a statewide outreach and enrollment program known as Take Care Utah. As the largest navigator grantee in the state of Utah from 2013 to 2018 and as a lead grantee of the current 2019 Connecting Kids to Coverage grant, UHPP is confident that it will be successful in continuing this work in the upcoming 2022 grant cycle. In Utah, most of the uninsured are concentrated along the urban Wasatch Front. For the purposes of this grant, UHPP will target the low-income, uninsured Hispanic population located in that geographic area consisting of Salt Lake, Grand, Weber, Davis, Utah, and Tooele counties.
As the lead grantee, UHPP will continue to oversee the work of the health access assisters, community outreach leaders, and counselors within Take Care Utah while increasing our capacity to serve more Utah children with the funds from this grant opportunity. In addition to the UHPP, four CBOs have been selected based on their success in outreach and enrollment during previous grant cycles and trusted presence in target communities. These subgrantees include Alliance Community Services, Community Health Connect, Health Access Project, and Utah Community Action. Each subgrantee’s efforts will build on their areas of expertise and credibility with low-income Hispanic families. These CBOs will draw on proven and imbedded outreach and enrollment strategies disseminated through UHPP and the larger network of Take Care Utah. Our project director will ensure all health access assisters are prepared to comply with the goals of enrolling 2,821 children in Medicaid or CHIP, 689 parents in Medicaid, and 201 pregnant individuals in Medicaid over the course of the three-year grant period.
UHPP is requesting $499,993 in year one, $497,700 in year two, and $497,700 in year three for a total of $1,495,393 to support these outreach and enrollment efforts.
Beyond the subgrantees, UHPP will continue to engage additional non-funded key partners including schools, Latino/Hispanic media outlets, local chambers of commerce, correctional centers, community health clinics, local health department programs, urgent care clinics, primary care settings, direct service providers, religious organizations such as the Church of Jesus Christ of Latter-Day Saints (Mormons) and congregations that predominantly consist of communities of color, WIC clinics, crisis nurseries, social media partners, tax assistance sites, federal and state legislative staff, Governor Cox’s office, refugee organizations, and billboard companies.
