Commonwealth of Pennsylvania, Department of Human Services, School Based Services grant. - The Office of Medical Assistance Programs, within the Department of Human Services (Department), operates Pennsylvania’s School-Based ACCESS Program (SBAP) in cooperation with the Pennsylvania Department of Education. The Department is pursuing this grant opportunity to support its efforts to move from the implementation phase of school-based services to the expansion phase of school based services and allow us to further support Local Education Authorities (LEA) in the provision of health-related services to students beyond just those with Individualized Education Plans (IEP) and Individualized Family Service Plans (IFSP). Utilizing stakeholder input and needs assessments, the Department will identify program needs as well as existing barriers and work with LEAs to expand health-related services to students beyond just those with IEPs and IFSPs.
