The Integrated Care for Kids (InCK) funding from the Centers for Medicare & Medicaid Services (CMS) provides an unprecedented opportunity for the City of New Haven to rally together to reduce the multiple physical, mental and social determinants of health that can be barriers to wellness for our citizens, in particular children 0-21 and pregnant women. InCK will support activities to address priority health concerns for children, including mental health and opioid and other substance use, and will improve the lives and outcomes for children and pregnant women who are eligible for Medicaid and CHIP. While New Haven has a wealth of assets that that can form the foundation of health and wellness, the city still struggles to address the root causes of health equity and economic stability. The InCK program will work to create ease of access, a single point of contact for families and care providers, and coordinated care for children and families in New Haven. Clifford Beers, with its expertise in trauma, its ability to form partnerships, its knowledge of integrated care coordination, its 105-year history of providing services in New Haven, and its client-centered and family focused approach, was chosen by the State of Connecticut to be its partner and leader of the InCK grant implementation. The Connecticut Department of Social Services (DSS) and all state child-serving agencies have pledged their support and cooperation for the InCK grant and it is anticipated that the successes generated in New Haven through this program will provide the foundation for a statewide rollout in the future. To achieve these goals, the InCK program will 1) Bring together New Haven health and community leaders, families, and child-serving educators and agencies; 2) Create a hub to help streamline, navigate, and integrate care for families; 3) Develop an integrated and coordinated data system leveraging existing state agency data and capabilities; 4) Identify and coordinate services for families, children, and pregnant women through a risk-stratification process based on need; 5) Find and engage families in their neighborhoods and at their schools; 6) Create a provider referral platform and a client platform; 7) Work with communities, leading community education and systems change processes; 8) Support our community partners to provide care coordination, including client-centered care planning, linkages to social services and supports, multidisciplinary team meeting development of natural supports, and other wraparound services; and 9) Create a sustainable per member per month care coordination payment model for individuals that have been determined to be at risk for poor healthcare outcomes. The proposed project has a first year budget of $3,000,000, which will be used to hire the program staff consisting of program leadership, Service Integration Coordinators (SICs), clinical, research, and informatics staff; create the data integration and provider and client platforms; and hire consultants for geo-spatial mapping, community mapping, education and community systems change.
