Aspire Indiana, Inc. proposes, "Improving Substance Abuse Outcomes Through the use of Health Information Technology," a project designed to enhance and expand coordination of care (e.g., substance abuse treatment) through the use of health information technology in targeted areas of need. The populations of focus consist of adults who are dually-diagnosed with a serious and persistent mental illness (e.g., schizophrenia, bipolar disorder, major depression) and substance abuse disorder, as well as adolescent substance abusers, who live in Boone, Hamilton, Madison, or Marion county, Indiana. Services to be provided will include intensive case management (this will include linkage with needed services such as primary care, mental health care, HIV care coordination, housing, food, transportation, childcare, and vocational services), and individual and/or group therapy. Services will be provided via teleconferencing if necessary, to remove financial andtransportation barriers to treatment. This project will serve 70 individuals on an annual basis, and 210 over the three-year project period.
