Building on its existing integrated care and wellness services, Southern California Health and Rehabilitation (SCHARP) PBHCI will improve health outcomes for adults with SMI and those with co-occurring substance use disorders, primarily minorities who are homeless, veterans recently incarcerated, with trauma history and/or HIVIAIDS in disproportionally affected Los Angeles County (LAC) Department of Public Health Service Planning Area 6 (SPA6), South Central LA. Demographics/Characteristics: racial/ethnic minorities experiencing health disparities, individuals with HIV/AIDS and Hepatitis A, B, & C, those affected by trauma histories and veterans and their families with a priority on those who are not currently receiving primary care. Adult consumers with SMI are diagnosed with 24% Schizophrenia, 28% Schizoaffective Disorder, 21% Bipolar Disorder, 15% Major Depression, 16% PTSD and 25% Co-occurring disorders (COD); 72% are African American, 20% are Hispanic/Latinos who speak primarily Spanish, 6% are White and 2% identify as two or more races; 47% are male, 40% are female and 13% are transgender; 83% identify as heterosexual, 3% as Lesbian, 7% as Gay and, 7% as Bi-sexual. Nearly 1/3 (29.3%) are defined as homeless, including 100% of the 90 Veterans and 50% of Assertive Community Treatment (ACT) clients. The main chronic health conditions experienced by consumers include cardiovascular disease, hyper-tension, asthma, diabetes and HIV. Annually, the project will serve: 75; 188; 300; 400 with a total of 400 over the project lifetime, exceeding the required minimum.
