Chronic disease and related modifiable risk factors such as physical inactivity, poor diet, tobacco use, and obesity are clearly contributing causes of death and disability in New Jersey. The New Jersey Department of Health has an extensive and successful experience in the provision of free breast and cervical cancer screenings for low-income, uninsured and under-insured women. In addition to screenings, this comprehensive program provides patient education, follow-up and support services, case management services and navigation into treatment. WISEWOMAN enrollees will be recruited from among age-appropriate New Jersey National Breast and Cervical Cancer Early Detection participants. The program anticipates enrolling 3,250 women in year-one of the project period. Services to be provided include a cardiovascular risk assessment; baseline screenings for blood pressure, cholesterol, glucose, height, weight, body mass index; and referrals, as indicated for medical follow-up for abnormal screening results and to evidence-based lifestyle intervention programs that include nutrition education, smoking cessation, and physical activity programs. All WISEWOMAN Program participants will receive cardiovascular risk reduction counseling. Culturally, linguistic appropriate and anticipatory guidance will be offered with a referral for evidence-based lifestyle interventions to promote optimal health and improve overall well-being.
