RAC has been implementing the Cooperation Agreement (CoAg) with CDC since 2017. The country has made a significant progress in testing and treatment coverage. Thus, over the period from 2017 to 2021, the number of PLHIV who know their status increased to 82% of the estimated number of PLHIV; treatment coverage is 66% of the number of PLHIV who know their status, and the achievement of viral suppression is 89% of the number of PLHIV, receiving Antiretroviral therapy (ART). We see the biggest gaps in the second 90s in the Kyrgyz national cascade. There are lot challenges, e.g., a lot of PLHIV stop taking ARVs. The main reasons for interrupting ART are external migration, difficult social living conditions, drug and alcohol addiction and internal migration. The objectives of the application will be to: 1) Assure high quality of HIV testing, linkage to care, treatment and viral load testing services, including ART, opportunistic infections (OI) diagnosis, treatment and prevention with focus on tuberculosis (TB) and sexually transmitted infections (STIs) and viral hepatitis (VH); 2) Provide high quality comprehensive HIV prevention to KPs; 3) Increase availability and use of quality data through routine monitoring and evaluation, and surveillance, including case-based and recent HIV infection surveillance and BBS to support quality of HIV program planning and implementation. The key expected outcomes follow the continuum of HIV services by the end of the project period are: increased coverage with high quality HIV prevention services (PrEP, PEP, sexual and reproductive health); improved level of knowledge and skills of medical staff on HIV prevention, care and treatment and effectively serving high risk populations; improved integration of HIV services with related services in the healthcare system (e.g. MAT, TB, STI services) including the Medical Health Insurance Fund (MHIF) activities; improved quality of HIV services via GSM throughout the HIV services continuum; increased ART coverage and adherence to treatment among the target population groups; improved and optimized HIV testing in compliance with international standards; increased number of HIV testing sites providing HIV RT in quality assured manner and able to distribute HIVST to clients and their contacts; increased PLHIV number of who receive quality-assured HIV care and treatment services; improved quality of HIV program data; established an electronic information data management system for HIV monitoring, which was integrated with other computer information management systems with wide range of data analysis capabilities; increased capacity of RAC staff to implement and monitor HIV National Program and coordinate activities of all organizations/stakeholders providing HIV services; improved the strategic information management efficiency; improved planning, implementation, monitoring and evaluation of HIV programs at the national and subnational levels; increased ART coverage with viral load testing coverage (VLC) and VLS rates; reduced HIV-related mortality; enhanced capabilities of institutions and the workforce to execute core public health functions of disease surveillance, response, including the associated laboratory and information systems.
