UNAIDS has defined five main Key populations (KPs) at high risk of HIV infection, including men who have sex with men (MSM), sex workers (SWs), transgender people (TG), people who inject drugs (PWID), and prisoners. KPs and their sexual partners account for 54% of the global new HIV infections and 25% of new infections in Uganda. Yet they lag in access to comprehensive HIV services and are difficult to reach due to underlying human rights barriers. Comprehensive HIV KP interventions are complex and require strong institutional governance and organizational capabilities which many CSOs and especially the KP-led organizations may not possess. This application will strengthen CSO capacity, coordination, and partnerships for accelerated HIV epidemic control through supporting comprehensive HIV prevention and treatment for KP to increase demand and access to high quality KP services and contribute to HIV epidemic control. The project will employ a collaborative approach with Ministry of Health (MOH), CDC, Implementing partners (IPs) and the KP consortium among other stakeholders. We will conduct a mapping and selection of the CSOs and capacity assessments to inform adaptation and development of capacity building tools. The capacity building will focus on four inter-related components: a) capacity building to improve CSO capacity to deliver quality health services; b) sub-award management and compliance training; and c) organizational capacity including governance, and d) advocacy and resource mobilization including grants writing. Our target is support 60 CSOs over the five year period. The capacity building will be predominantly hands-on, tailored to each CSO needs, and will utilize real challenges identified in the capacity assessments for hands-on support, with tangible outputs such as improved tools (e.g. strategic and operational plans, service delivery algorithms, and compliance tools). An outline of the areas for improvement for each CSO will be mapped out in a work plan with timelines for follow-up support and improvement. Face-to-face and e-learning communities of practice will be established for exchange learning. The same approach will be used to tackle the human rights barriers, in addition to providing legal services to CSOs as needed. To support implementation and utilization of HIS solutions we will build on the Crane Survey and the MakSPH monitoring and Evaluation Technical Support (METS) experience, to integrate KP tools into the innovative audio computer assisted self-interview (ACASI) to support identification, classification, reporting, linkage of KPs to targeted prevention services, car e and treatment service and generate data which are linked and reported through KP tracker and HIS. The Crane Survey team will train and support CSOs and IPs to integrate these tools. MakSPH (lead organization) has constituted a consortium of three institutions (MakSPH, PACE and HIRAPF) with wide experience in KP service delivery, organizational capacity building and KP human rights/legal services to implement this project. The Most-at-Risk Population Initiative (MARPI) will also provide support in service delivery capacity building. Building on our consortium experience, across all the five strategic areas, we propose to apply the three constructs of demand—supply—support theory of change, to support CSOs to rapidly increase demand for (by addressing underlying human rights barriers and other bottlenecks) and provide quality KP services. The support will be consultative using human centered approaches at various levels including, affected communities, partners in service delivery (CSOs, CDC IPs) and policy (MOH and Uganda AIDS Commission), other national stakeholders including cultural and religious leaders, CDC, among others. Our target is to raise the percentage of number of KPs retained in care from 60% to 95% over the 5 year period. This project builds on more than 12 years of the MakSPH ongoing KP surveillance and service
