Over the last twenty years Kenya, through support of its partners, has intensified investment towards HIV epidemic control by increasing access to antiretroviral therapy (ART), implementing effective prevention strategies including voluntary medical male circumcision, prevention (VMMC), prevention of mother to child transmission and HIV and STI prevention) According to the 2018 estimates, the current national HIV prevalence is at 4.9%. The epidemic is still proportionately distributed among women with 6.2% national prevalence compared to 3.5 among men and adolescents and youth contribute 49% of all new infections NASCOP presentations, 2018). According to MOT 2008, key populations (KP’s) including sex workers, men who have sex with men (MSM) and people who inject drugs (PWID) represent less than 2% of the general population but they contribute a third of all new HIV infections. Prison populations are disproportionately affected by HIV (NASCOP 2014). LVCT Health, a local Kenyan organization registered in 2002, not only meets criteria (2) of PEPFAR’ local partner definition in response to this RFA but has been a key part of the national HIV response and contributed many innovations and technical support for national scale up including experience working with the Kenya Prisons Services (KPS). The main project goal will be to reduce HIV infections among the Kenyan prison population, reduce mortality and morbidity of HIV-positive inmates, prison workers, and family members, and strengthen systems and build capacity of KPS to implement HIV programs sustainably, while the capacity building goal will be to strengthen systems and build capacity of KPS to take lead in planning and implement the HIV response within the prisons set up. The project will have 3 main objectives: 1) Increase identification and linkage of PLHIV and increase access and uptake of HIV care and treatment and TB management among the prisons community by 2024 2) Strengthen the KPS health system to provide optimal HIV combination prevention services to more than 95% of prisoners, KPS staff and their families and surrounding communities by 2024. 3) Strengthening systems for KPS and CHMTs in counties with prison facilities on delivery and management of high-quality HIV care and treatment services. Key strategies will include: 1) Capacity building the Kenya Prisons Service staff and program management systems for them to be able to efficiently manage the program and continually ensure integration and health provider capacity for program scale up and sustainability. 2) Foster working relationships between county governments and KPS health systems by advocating for a legal relationship with the Council of Governors (COG) and implementing it in each of the counties we will work in.
