In this funding request, Global Health Systems Solutions (GHSS) will assist the government of Cameroon to strengthen and expand laboratory systems and services, and build the competencies of laboratory staff in order to improve the quality of testing needed to support HIV prevention, treatment and care goals in prioritized national and sub-national geographic locations of Cameroon that have been prioritized for intervention by CDC/PEPFAR. GHSS will leverage its vast experience acquired over the last 5 years, as the sole CDC/PEPFAR laboratory-implementing partner, to implement the activities outlined in this application and address existing challenges that hinder access to and availability of quality laboratory services. GHSS specific objectives for the Request for Funding Application (RFA) will include: 1) implementing Quality Management Systems geared towards laboratory accreditation using innovative quality strategies; 2) implement and expand External Quality Assurance programs to support testing for HIV serology, Early Infant Diagnoses, viral load, CD4 and Tuberculosis, continuous Quality Improvement; 3) develop a specimen referral networks to reduce turnaround time and improve referral of specimens across PEPFAR-supported sites; 4) provide technical assistance and support training and mentoring of ministry of health laboratory personnel to ensure institutionalization and sustainability of Continuous Quality Improvement; 5) provide technical assistance to support implementation of Basic Laboratory Information Systems (BLIS); 6) build capacity of the National Public Health Laboratory to support coordination of diagnostic services and networks; and 7) provide training of equipment maintenance and calibration officers and establish service maintenance contracts to laboratories to be accredited. At the end of the project period, GHSS expects to achieve the following outcomes: 1) improve the quality management systems of 65 laboratories according to the WHO AFRO Stepwise Laboratory Improvement Process Towards Accreditation (SLIPTA) and accredit at least five of them in the CDC/PEPFAR prioritize locations, to international standards, resulting in improved accuracy and reliability of patient test results; 2) expand coverage and improve the quality of HIV test results by certifying 600 testing sites and testers, using the WHO-CDC Stepwise Process for Improvement of Rapid Test standards and 50 HIV-related point of care testing sites engaged in testing for Early Infant Diagnosis, viral load, CD4 and TB; 3) establish 10 effective specimen referral networks that will help decrease by 80% turnaround time from specimen collection and reporting of patient test results in the CDC/PEPFAR priority areas, leading to better patient management; 4) train and develop competences of 1,550 ministry of health laboratory personnel in the areas of continuous quality improvement to ensure institutionalization and sustainability of laboratory efforts in Cameroon; 5) establish BLIS in all 65 laboratories enrolled in SLIPTA; with the expectation that BLIS will help reduce by 70% the delay of reporting of patient test results and reduce error rates by 90%, and enhance overall retention in care; 6) support the development of the capacity of the National Public Health Laboratory (NPHL) to coordinate and link all 10 Regional Hospital laboratories in the country; enhance the NPHL’s capacity to provide leadership and oversight in implementing Quality Management Systems across the Regions of Cameroon by developing policies and guidance to ensure increase proportion of laboratories receiving HIV and point of care proficiency testing with follow-up corrective actions; and 7) provide training of equipment maintenance and calibration officers and establish service maintenance contracts with five laboratories to be accredited; as well as validate test methods in the same laboratories.
