HEALTH CARE NEEDS/BARRIERS TO BE ADDRESSED. Since 1971, Lincoln Community Health Center (LCHC) has provided high-quality, affordable primary health care services to the residents of Durham County, North Carolina. LCHC also serves the homeless special population. The low income, lack of insurance, and lack of providers in the region contributes to health outcomes, indicators, and mortality rates worse than the nation. For instance, these include adult diabetes prevalence (7.2%), adult obesity prevalence (28.3%), colorectal cancer mortality (13.2), breast cancer mortality (22.1), infant mortality (6.3) low birth weight (8.8%), mental/behavioral disorder mortality (62.8), and HIV infection prevalence (0.6%) (CARES Engagement Network, University of Missouri Extension, 2020; WONDER, CDC, 2014-2018). PROPOSED PROJECT AND SERVICES. Services provided either directly or by referral include primary care, diagnostic labs, radiology, screenings, emergency and after-hours coverage, voluntary family planning, immunizations, well child care, gynecology, prenatal care, labor and delivery, postpartum care, preventive dental, pharmaceutical services, HCH required substance use disorder services, case management, eligibility assistance, health education, outreach, transportation, translation, additional dental, mental health services, optometry, nutrition, psychi
atry, ophthalmology, dermatology, and nephrology. In 2020, LCHC served 34,652 patients with 102,127 (including 20,270 virtual) medical, 8,304 dental, 10,363 (including 1,087 virtual) mental health, 113 substance use disorder, 307 nutritionist, 303 vision, and 3,907 enabling encounters. This includes 353 homeless clients (UDS). LCHC operates nine service delivery sites in the county. One is an HIV Early Intervention Services site, one is a Healthcare for the Homeless site, and one is a School Based Health Center. In Year 1, staff will include 18.20 FTE physicians, 18.23 FTE midlevel providers, 23.0 FTE nurses, 56.0 FTE other medical staff, 4.0 FTE dentists, 1.0 FTE dental hygienist, 5.0 FTE dental assistants, 1.0 FTE psychiatrist, 5.7 FTE LCSWs, 0.13 FTE ophthalmologist, 1.0 FTE pharmacy personnel, 11.0 FTE case managers, 2.0 FTE education specialist, 2.0 FTE outreach staff, 4.0 FTE transportation staff, 7.0 FTE interpretation staff, 2.0 FTE Community Health Workers, 1.0 FTE QI staff, and 30.3 FTE other programs staff (Nutrition, WIC, SAMHSA). From 01/01/2023-12/31/2023, LCHC projects serving 32,756 patients. COMMUNITY/TARGET POPULATION GROUPS TO BE SERVED. The service area has a population of 306,457. Of residents, 33.2% live with income below 200% of the Federal Poverty Guideline (FPG), and 16.0% live with income below 100% FPG. Of residents, 12.6% are uninsured, while 23.4% of residents living with incomes below 200% FPG are uninsured. The population of the service area is 51.4% white, 37.0% African American, and 4.9% Asian (American Community Survey, Census Bureau, 2014-2018). A point-in-time count found a total of 349 homeless individuals in the county (“Homeless Populations and Subpopulations,” HUD Continuum of Care Homeless Assistance Program, 2021). However, this is a one-night count, and actual numbers of homeless in the area are much higher.
