Health Center Program - History of organization: Fish River Rural Health (FRRH) began offering health care services to the most remote communities of Northern Maine in Aroostook County in 1978. Aroostook County consists of 6,453 square miles, over 4 million acres of land with 86% covered by forest. It is the largest county east of the Mississippi and is larger than the states of Connecticut and Rhode Island combined. From 1978 to 1990, FRRH received HRSA funding through the Aroostook County Action Program (ACAP), the local antipoverty community action agency for all of Aroostook County. In July 1990, FRRH became a 501(c)(3) nonprofit corporation, applied for, and subsequently received independent 330 Grantee status. Groups served: Since its inception, FRRH’s target population has been the most vulnerable members of the community: people whose income is at or near poverty level; the elderly and isolated; persons without health insurance or adequate coverage; and individuals with multiple chronic ailments. The FRRH service area consists of twelve towns, two plantations and 43 Unorganized Territories (pop. approximately 15,000) spread over the northernmost county in Maine. FRRH is the only FQHC in the region and the only health center that serves low income and underserved individuals and families and maintains a sliding fee scale. Summary of health care needs: The health care needs are overwhelming including high rates of multiple chronic diseases as well as significant risk factors. Prevalent health factors include diabetes, cancer, mental health disorders, respiratory and cardiovascular conditions. In addition, the region’s vast geography and lack of public transportation increases isolation and depression. Compounding the situation is the area’s severe poverty, economic struggles and high unemployment. At the same time, given the very rural nature of the service area, access to care is a major challenge. All service area municipalities are designated Medically Underserved Areas (MUAs) and all towns are Health Professional Shortage Areas (HPSAs) in all three categories: medical, dental and behavioral health. Service delivery: FRRH operates health centers in three St. John Valley communities and mobile services to seven area schools, in an effort to reduce transportation barriers. Service delivery clinic sites are located in Eagle Lake, Fort Kent, and Madawaska, Maine. Services are comprehensive and ensure that continuity of care is provided. The practices are based on “family practice” principles and treat the whole family from perinatal to geriatric. FRRH sites directly provide primary medical, dental, behavioral health, substance use disorder counseling, biofeedback, psychiatric medication management, RN care coordination, wellness, food and nutritionist services, foot care, optometry, chiropractic care, and pharmacy. FRRH is a NCQA-deemed Patient-Centered Medical Home, PCPlus Tier III Value-Based Practice, and Accountable Care Organization participant. For the projected period of April 1, 2026 through March 31, 2030, FRRH anticipates meeting Service Area Announcement Table target of serving 8,568 patients annually. FRRH is applying for continuation of 330 grant funding under opportunity number HRSA-26-004 and requests $2,234,067 in federal funding, which is critical to sustain these rural health services.
