Existing infrastructure in the Denver-Metro area has proven insufficient to fill gaps between the community's homeless crisis response system, affordable housing options, and other community resources necessary to effectively serve the rapidly increasing population of persons experiencing homelessness. VOACO will utilize the GBHI program to provide direct services in support of efforts to develop and expand of the Denver-Metro community's infrastructure to integrate substance use disorder treatment, housing services, and other critical services for individuals, youth, and families experiencing homelessness. Priority for access to services will be given to individuals and families experiencing chronic homelessness, and those coping with substance use disorders, serious mental illness, serious emotional disturbance or co-occurring mental and substance use disorders. VOACO will utilize evidenced based practices including Housing First, Motivational Interviewing, Trauma-Informed Care, Cognitive Behavioral Therapy, Harm Reduction and Critical Time Intervention, to orient case management and recovery-oriented systems of care. The VOACO GBHI program will serve 60 high-vulnerability individuals annually, 300 over the course of the five-year grant, and will support the expansion of local implementation of substance abuse treatment, housing services, and case management supports within the community. Service delivery will include navigation to permanent housing, behavioral health treatment for mental health and substance use disorders, access to SSI-SSDI through SOAR practitioners, peer recovery support, and enrolment in Medicaid or other eligible healthcare and public benefits. VOACO anticipates that participants will be connected through the local coordinated entry system, at least 75 percent of GBHI clients will have obtained permanent housing, at least 75 percent of GBHI clients exited into permanent housing will have retained their housing for a period of at least one year post-exit, and 100 percent will have access to behavioral health services. Under an existing Cooperative Agreement for the Benefit of Homeless Individuals grant award, VOACO, over the last 2.5 years, has demonstrated an exceptional record with an intake coverage rate of 109 percent and six-month follow-up rate of 80.6 percent (SAMHSA, 2019). VOACO met all implementation milestones for the CABHI grant and anticipates no issues in meeting all GBHI goals and measurable objectives.
