Hyacinth AIDS Foundation (HAF), founded in 1985 to help provide a quality of life for those dying from AIDS, embraces a current mission to help individuals live with HIV, stop the spread of the epidemic and serve as a critical voice in the public debate surrounding HIV. With seven sites in six of New Jersey’s largest urban centers, HAF has the capabilities in infrastructure, expertise and capacity to effectively meet the requirements outlined in this proposal. The proposed program will be housed at Hyacinth’s Project LOL (Live Out Loud) site in Jersey City. Project LOL has been funded by the CDC since 2011, and provides HIV testing, connection to medical care, and prevention services to young men who have sex with men, ages 13-29, while implementing several evidenced based interventions geared towards this population. Target Population and Catchment Area: Hyacinth proposes to target Black/African American and Latinx men who have sex with me ages 13-29 in the catchment area of Hudson County (New Jersey), which has been identified by the Federal Department of Health and Human Services as one of the localities hardest hit by the HIV epidemic. All year 1-year objectives will be completed by August 31, 2021. 1a- Develop and disseminate communication messages reaching 750 individuals; 1b- Enroll 75 Black and Latinx GBM ages 13-29 in Hudson County through motivational interviewing sessions; 1c- Assess 75 (100%) Black and Latinx GBM, ages 13-29 enrolled in motivational interviewing for substance misuse (DAST-10) and HIV, HCV and other STIs. 1d- Assess 100% of participants enrolled in motivational interviewing for health screening and provide referral and navigation services, as appropriate and Assess 100% of participants enrolled in motivational interviewing for health screening and provide referral and navigation services, as appropriate; Objective 1f: 10 educational training sessions will be held for educators and other professionals who work with the target population to address importance of HIV screenings and social determinants of health. The program will utilize the Trans-Theoretical Model (TTM) , Motivational Interviewing (MI) and HIV Navigation Services (HNS) to fulfill the program goals and objectives.
