Special Diabetes Program for Indians will serve all Native Americans individuals at the White Eagle Health Center who have been diagnosed with diabetes or are at risk of developing diabetes. - The purpose of the Special Diabetes Program for Indians is to provide diabetes treatment and/or prevention activities and/or services for AI/NA communities. The Ponca Tribe of Oklahoma Diabetes Program is located in the community of White Eagle, Oklahoma, approximately five miles south of Ponca City, OK. The Ponca Tribe of Oklahoma Diabetes Program will focus on diabetic ages 17-56 years to help generate self care and disease process awareness by participation with the Diabetes Program' educational classes, walking events and cooking classes presented by the Diabetes Program's Registered Dietitian. The Ponca Tribe of Oklahoma Diabetes Program has selected Diabetes-related Education for the Best Practice. Incentives will be offered for diabetes who have completed yearly exams such as labs, complete foot exam using the 10 gram filament, dilated eye exam, and EKG. A list for diabetic supplies will be generated and blood pressure and blood glucose monitors will be given for diabetic self monitoring and maintaining a record of results to report on doctor visits.pic.
