Since 1989, AccessMatters, has brought together pediatric, infectious disease, primary care, and social service organizations to create a network that would provide coordinated, comprehensive, culturally and linguistically competent services to WICY with HIV. WICY served: AccessMatters’ Part D Program, the largest in Philadelphia, has been an integral part of the HIV healthcare delivery system in the Philadelphia Eligible Metropolitan Area (EMA). In 2019, 2020, and 2021, AccessMatters’ Network served 1,075; 1,287; and 2,821 WICY living with HIV, respectively. Service Area: AccessMatters’ Part D Program includes a Network of sub-recipient providers located throughout the city and will serve the City of Philadelphia, inclusive of all zip codes. WICY populations served: Specific WICY priority populations to be served through tailored services and evidence-based interventions include Black women, transgender women, youth aged 13-24, and people who inject drugs. Key Services and Quality Improvement Measures: AccessMatters’ Part D Program improves access to primary HIV medical care for WICY living with HIV through the provision of Core Medical Services (O
utpatient/Ambulatory Health Services, Mental Health Services, Medical Nutrition Therapy, and Medical Case Management, including Treatment Adherence Services) and Support Services (inclusive of psychosocial support services, housing, food bank/home delivered meals, child care services, and more). AccessMatters delivers these critical services through contractual partnerships with a network of well-established and respected medical and support service providers. RWHAP support services are also provided to affected clients as part of a family-centered model of care. AccessMatters’ central role within this network is to ensure the quality of clinical and support services and the capacity of its Network to respond effectively to changing needs of consumers, changing healthcare systems, and advances in HIV prevention and treatment. AccessMatters has a robust quality improvement program that prioritizes retention in care and viral suppression, the two care continuum stages that are the greatest barrier in Philadelphia to ending the HIV epidemic. Supplemental funding: AccessMatters requests $150,000 to support development and implementation of a comprehensive, streamlined implementation guide for transitioning youth into adult HIV care. This activity aims to address client needs on the HIV care continuum related to linkage to care, receipt of care, and retention in care.
